Mifepristone is a selective progesterone receptor binding modulator with primary antagonist properties.
It binds to endometrial progesterone receptors minimally estrogen receptors and up regulates androgen receptors.
Reduction in the fibroid size with mifepristone is due to direct effect in reducing the number of progesterone receptors.
Morphy et al reported mifepristone 25 mg to be the most effective dose to cause clinically significant decrease in leiomyoma(fibroids).
Long term use of high dosage of anti progesterone may promote an unopposed estrogen leading to endometrial hyperplasia.
